It can be done if your phone company lets you establish a 3 way call before the 3rd party answers the call.

 

Your voice modem would need to also issue the correct length hookflash which is correctly recognised by your phone company.

My phone company does not let me establish a 3 way call untill the third party picks up. Is there any inexpensive way of establishing a 3 way call without having to prompt the third caller to press a key? Is there a voice modem which can handle that? Or maybe an inexpensive dialog card? Or maybe some software which can detect the third pardy picking up the phone? Or any other Ideas?

You will need to use a Dialogic card capable of "Dial and Conference".
